To request a copy of your HUP transcript:

• Call (215) 615-2625, or
• Send a written request to:

  Philadelphia, PA 19104

With a written request, please include the following:

• A written request for the specific record desired;
• Authorization to release same name, address, and title of person to whom it is to be sent; and
• Include a check or money order in the amount of $2.00 for each transcript, made payable to The Hospital of the University of Pennsylvania
