Education Verification

Employers wishing to complete an education verification for a student must send the verification form AND a release signed by the student via fax to 215-898-7399 or scan/email to osi@nursing.upenn.edu.  Verifications cannot be completed without the signature of the student or alumna/us.  Please include the student's name at time of attendance.

Note: For graduates of Planned Parenthood, please contact 212-541-7800 (Sandy Worthington).

Transcripts

Official transcripts are generated from the Office of the University Registrar and may be requested online. 

Students who graduated from Chestnut Hill, Lankenau or St. Joeseph's Schools of Nursing should contact the Barbara Bates Center for transcripts.

Students who graduated from the Hospital of the University of Pennsylvania (HUP) School of Nursing should contact UPHS Human Resources for transcripts.
