REQUEST FOR PHOTOCOPY AND PHOTOGRAPHIC SERVICES
Reproductions of materials in the Barbara Bates Center for The Study of The History
of Nursing are provided as a service to expedite research and lessen wear on
documents, and are made solely for the personal use of the individual researcher
requesting them. Reproductions may not be transferred to another individual or
organization, deposited at another institution, or reduplicated without prior permission
of the Center. Duplication by the Center in no way transfers either the copyright or the
property right, nor does it constitute permission to publish in excess of "fair
use" or to display materials. In some cases the Center may refuse
to honor reproduction requests because of the physical condition of the material, the
amount of staff time required to do the work, special restrictions imposed by the donor,
copyright law, or right-to-privacy statutes.
To request a photocopy or photograph:
1) Read the copyright warning below and sign your name.
2) Complete the order form, being sure to indicate which specific documents you are
requesting to be copied.
3) Return this form with appropriate payment to the Center, along with a statement
indicating for what purposes the copy will be used.
Orders will be filled as staff time permits. Generally, photocopy orders are filled
within three to five working days. Photographs generally take seven to ten day to
complete. Rush service is available at additional cost. Orders may be held for pick-up or
mailed. There are additional fees for mail orders.
NOTICE: WARNING CONCERNING COPYRIGHT RESTRICTIONS
The copyright law of the United States (Title 17, United States Code) governs the making
of photocopies and other reproductions of copyrighted material. Under certain conditions
specified in the law, libraries and archives are authorized to furnish a photocopy or
other reproduction. One of the specified conditions is that the photocopy or other
reproduction is not to be "used for any purpose other than private study, scholarship
or research," If a user makes a request for, or later uses, a photocopy or
reproduction for purposes in excess of "fair use," that user may be liable for
copyright infringement.
This institution reserves the right to refuse to accept a copying order if in its
judgement, fulfillment of the order would involve
violation of copyright law.
