Student Transcripts

The Barbara Bates Center for the Study of the History of Nursing provides a student transcript service for graduates of a number of schools of nursing that are no longer in operation and for which the Center holds the student records. The Bates Center provides student transcripts for the following schools only:

Chestnut Hill Hospital School of Nursing (please contact Theresa McMenamin - 215-248-8200 directly for transcripts)

Lankenau Hospital School of Nursing

Mercy-Douglass Hospital School of Nursing

St. Joseph’s Hospital School of Nursing. Lancaster, PA.

Temple University Health System – Episcopal Hospital School of Nursing & Perfusion Science

Graduates of the Presbyterian Hospital School of Nursing Please Note

Graduates of the Presbyterian Hospital School of Nursing RN and LPN programs should contact the Nursing Education offices at the Penn Presbyterian Medical Center to obtain transcripts.  The contact number is 215-662-9165. 

Graduates of the Pennsylvania Hospital School of Nursing Please Note

All archival materials from the Pennsylvania Hospital are housed at the Historic Pennsylvania Library and Archives.  For more information, please contact Stacey Peeples, Curator & Lead Archivist at 215-829-5434

Graduates of Schools of Nursing not listed above

If the school from which you graduated is not listed above as one of the schools for which the Bates Center provides student transcripts, the Center does not provide a student transcript service. Those individuals should contact either the affiliating hospital or the alumnae association connected with the school of nursing.

Who may request  a student transcript

Individuals whose name is listed on the transcript may request a transcript. Requests from third parties, such as hospitals, educational institutions, or other agencies, will be honored with a release of information signed and dated by the student authorizing the release of infomation, and receipt of payment.

Academic Certification/Verification

The Barbara Bates Center for the Study of the History of Nursing provides verification of dates a student attended and/or graduated from the schools listed above. To obtain academic verification please follow the process described below under “Ordering Transcripts.”

Ordering  transcripts

Request for transcripts must be sent to the Barbara Bates Center for the Study of the History of Nursing,University of Pennsylvania School of Nursing, Floor 2U, 418 Curie Blvd, Philadelphia, PA  19104-4217.

Please note that mailing your correct to the incorrect address will delay processing significantly. 

Please  enclose a check for $10.00 for each requested transcript payable to “The Trustees of the University of Pennsylvania". Please note that payment must be received with any transcript or verification request in order for it to be processed. To download the Center's Transcript Request Form click here. 

The following information should be included with any request:

• Name of school
• Years of attendance and/or year of graduation
• Name as it appears on the student record 
• Type of Transcript that is being requested: Official - in a signed, sealed envelope or Unofficial not in a signed, sealed envelope
• Day time phone number where you can be contacted if we encounter any problems processing your order
• Number of transcripts requested
• Student's signature to release transcript
• Address where the transcript(s) are to be sent
• For Temple University Health System – Episcopal School of Nursing transcripts:
• Please specify type of program from which you graduated, i.e., RN or LPN
• Also, please specify if program was the day or evening program
