Assessing
Health-Related Quality of Life
Among Frail Elders
Mary
D. Naylor, PhD, FAAN, RN and Kathy Bowles, PhD, RN
Since
1989, Dr.
Mary D. Naylor has led interdisciplinary programs
of research designed to improve outcomes and reduce costs of
care for vulnerable community-based elders. To date, Dr. Naylor
and her research team have completed three National Institute
of Nursing Research (NINR)-funded randomized clinical trials
focusing on discharge planning and home follow-up of high-risk
elders by advanced practice nurses. (see also, Transitional

Maintaining
continuity in patients' medical care is especially critical
following discharge from the hospital. Research shows that one-quarter
to one-third of these patients have to return to the hospital
for problems that could have been prevented.
Dr.
Kathy H. Bowles is developing a program of research
to improve health care for elders and examination of health
care provider’s decision-making regarding hospital discharge
referrals for high-risk elders. She has published and presented
on the use of nursing classification systems and information
technology to describe, inform, and influence the care of high-risk
elders. Her current NIH funded research focuses on developing
an expert decision support system for discharge referral decisions
at hospital discharge for older adults. Other research interests
include testing a comprehensive clinical assessment of the health
and quality of life of newly admitted older adults using a web-based
information network among several community-based agencies such
as retirement communities, nursing homes, and home care. Dr.
Bowles was clinical project director for a telehome care study
with diabetics, and Principal Investigator of two pilot studies
exploring the use of tele-video visits for homebound elders
with congestive heart failure.
