Innovations

 

Under the motto, "Leadership in Creating and Disseminating Innovations for Frail Vulnerable Elders", Penn Nurses are developing exportable innovations in care that are germane to national policy and system changes, including a multi-site demonstration of the transitional care model and translation of the nursing home palliative care model to homebound frail elders.

Selected PENN NURSING Innovations

LIFE" (Living Independently For Elders) is a Program of All-inclusive Care for the Elderly providing integrated acute and long-term care services for frail elderly who are nursing home certifiable and prefer to live in the community. As Dr. Lois Evans points out, "LIFE is a classic example of an innovative adaptation to other PACE models. We are the first and only school of nursing in the country to run a PACE program which also functions as a laboratory for research, interdisciplinary education and dissemination of new models of geriatric care.” >>> more

   
Transitional Care. Multi-site demonstration of innovative model of care coordination delivered by advanced practice nurses (APNs) for older adults making the transition from hospital to home.

Palliative care. A model of palliative care in community nursing homes was implemented, emphasizing advance care planning, pain and symptom management, and psychosocial support.

   
Telehealth Technology. Kathy Bowles and clinicians from the Visiting Nurse Association of Greater Philadelphia (VNA) ) have developed expertise in telehomecare through several research projects conducted at the VNA. >>> more
